If you live within our local intake area, an Enrolment pack can be collected from our Administration office. Please bring a current utilities invoice with you (not older than 3 months) as proof of your current address.
When returning your Enrolment paperwork, we also require copies of the following documents:
- Copy of your proof of address (must be a rates notice, utilities account or lease/purchase contract but cannot be older than 3 months)
- Copy of your child’s birth certificate
- Copy of your child’s immunisation history
- Copy of your child’s entry date into Australia (if not born in Australia)
- Copy of your child’s VISA documents (if not an Australian citizen)
In addition to the above documents, in order to assist with placement of your child in the appropriate classes/programs, it would be beneficial to provide copies of:

Recent legislative changes under the Public Health Act 2016 (WA) and School Education Act 1999 (WA) require parents to submit their child’s current Australian Immunisation Register (AIR) Immunisation History Statement as part of enrolment documentation. Under these new changes, the child’s immunisation status must be ‘up-to-date’ according to their AIR Immunisation History Statement. The statement must be dated within 2 months of the date of application for enrolment.
You can obtain a copy of your child’s statement by:
- logging into Medicare online via MyGov
- using the Medicare Express Plus app
- visiting a Medicare or Centrelink office, or
- calling the AIR General Enquiries Line on 1800 653 809
